Our health professionals obtain consent from clients to discuss and collect their personal information at the start of our engagement. We collect information that includes:
We may collect personal information from others such as ACC, Health NZ, your GP, other medical providers or a publicly available source. We will always collect your personal information in a way that is legal and reasonable.
We also may collect information about you from another source or person if you have given consent for this.
If we are required to collect information about a child, this should be provided by or with the consent of the child’s parent or guardian.
We only use your personal information in relation to provision of rehabilitation and assessment services or if it is necessary for us to meet legal and regulatory obligations.
If you do not consent for us to collect and use your personal information, we may not be able to provide you with our services.
We will only share personal information in relation to the purpose we collected it and with person/s or entity you have given your consent for us to do so or where there are other legal or compliance reasons.
Information may be disclosed where this is necessary to avert or minimise a serious threat to public health or safety, or to the life or health of any person.
We understand privacy is important and we are committed to keeping your information safe.
You have the right to access the health information we hold about you. You can request this by emailing admin@bayrehab.co.nz. We will need to ensure proof of identity before releasing any information to you.
If you think the information we have provided to you is inaccurate you are entitled to ask for it to be corrected. This request needs to be in writing outlining specifics of what information is considered by you to be incorrect.
This request will then be reviewed with the author of the report. If the information is factually incorrect and the author agrees, we will amend and update the information. If we are unable to make the correction we will inform you, and you can choose to provide a written statement of correction which we will add to that file which outlines the information you requested to be corrected.
